American Community Mutual Insurance Company has started selling the Community Flex product in Arizona and Indiana.
American Community, Livonia, Mich., already has been selling the preferred provider organization plan in 11 other states.
Community Flex PPO coverage can take effect in Arizona and Indiana starting March 1, American Community says.
American Community offers a choice of 3 different Flex plan designs, and it will guarantee rates for 24 months for consumers who agree to take a deductible of $5,000 or higher.
For deductibles of $500 to $3,500, the premium rate is guaranteed for the first 12 months of coverage.
The lifetime benefits maximum is $5 million per insured.
All options include an accident benefit and extra accidental death and dismemberment coverage.
